WHO WE ARE

 

Community Care Durham (CCD) is a multi-service registered charitable organization providing a broad range of community support services for adults and their caregivers who have needs related to aging, physical and/or mental health. We assist our clients with services that support their independence, health and quality of life. In doing so, we help them remain in their own homes – where they want to live.

CCD’s main funding support is from the Province of Ontario’s Ministry of Health & Long-Term Care through the Central East Local Integration Network (CE LHIN). In addition, funding support is received from the United Way of Durham Region, area Municipalities and our generous donors. See our “ Partners and Supporters” for more information.

Some of our services (i.e., Friendly Visiting, Telephone Reassurance, Mental Health support, Help getting home from Hospital, Assisted Living, assisted Transportation etc.) are offered free of charge. Other services (like Meals on Wheels, Luncheon Out, Foot Care, Personal Support, Day Program, Transportation, Exercise & Falls Prevention classes etc.) have a service fee in order to assist with the total cost of service delivery. Please  see individual services  for more information and fees where applicable.

Our offices are located throughout Durham Region in the communities of Ajax, Brock, Clarington, Oshawa, Pickering, Scugog, Uxbridge and Whitby.

The Community Food Table

In response to the current public health crisis, CCD has developed a new service, the Community Food Box. People can purchase boxes directly from CCD. If you wish to purchase a box for a specific person, call 1-888-255-6680 or donate a Food Box online!

Community Care Durham-Walk for Mental Health Awareness-2020

Celebrate the 15th annual CCD Walk for Mental Health Awareness. Walk or run 1, 2 or 5 km from the Fairgrounds to the Port Perry Waterfront Trail, then return for a complimentary lunch! June 13, 2020, 9a.m. to 12p.m., Port Perry Fairgrounds. See you there!
